Ages 6 months and older

The Centers for Disease Control and Prevention (CDC) approved updated booster doses of Pfizer and Moderna COVID-19 vaccine for people aged 6 months and up. People can receive an updated booster at least two months after a previous booster or completing the primary series. People can choose either updated booster regardless of the brand of previous vaccine doses.

Kids who received the Pfizer vaccine will get the updated booster as their last dose in the three dose series.

Moderna updated boosters for ages 6 to 17 years old are not available at the Health Department.

IF the vaccine recipient will be UNDER 19 on the date of receipt, AND a parent/legal guardian WILL NOT be present, completion of these consent forms is REQUIRED.
